·          Valid Signed Lease (required if applicable)

·          Utility Bills (gas, electric, phone, cell phone)

NOTE:  An appointment is required for registration.  Two (2) of the forms must be signed in the presence of a Notary Public before they are submitted to the Registrar.  The forms are the OWNER AFFIDAVIT and the RESIDENCY AND CUSTODY AFFIDAVIT.
