To the Parents of Ford Middle School:

All students interested in participating in sports at Ford Middle School will need the following items completed (ALL SIGNATURES NEEDED) and turned into the coach prior to the first day of practice or try-outs:

1. Emergency medical card – completed by your parent/guardian

2. Athletic Participation/Residency Verification/Release Form

3. Student Code of Conduct

4. Berea City School District Insurance Waiver

5. Physical – signed and dated by your doctor (good for one calendar year)

• Please make sure both front and back are completed.
• Please make sure that if your doctor uses a physical form other than the OHSAA physical, you must still fill out the front side of the OHSAA physical form and attach the two together.
• A physical form not signed/dated by your doctor/parent or guardian, or not fully completed will not be accepted by the coach.
• If your son or daughter has played a sport previously, please check with the Athletic Department to see if a current physical is on file.

There will be a participation fee per athlete per sport.There is NO family cap. If you play a sport that requires a try-out, please DO NOT pay until teams have been decided.
