Health Screenings

Basic screenings are provided during scheduled clinic hours. Required screenings include:

 

• Vision: Pre School, K, 1, 3, 5, 7, 9, 11 referrals, referred (no report received)
• Hearing: K, 1, 3, 5, 9, 11 referrals, referred (no report received) 

 

Screenings and referrals are completed in accordance with Ohio Department of Health guidelines.

If you are in financial need with no insurance for vision or hearing check-ups, please contact your School Health Specialist for available resources.

 

IMMUNIZATIONS

Students enrolled in kindergarten through grade 12 must provide written proof of immunizations currently required under Ohio law on file at their school. Ohio Revised Code 3313.671 requires schools to exclude students from attendance no later than 14 school days after admission if they are not properly immunized or have an exemption form on file in the school clinic. Remember to update the clinic with any new immunizations received. Please send a copy of the updated record that includes the month, day and year of immunization.
