Transportation Forms
Please see the list of forms below. Utilize the appropriate form based on the information you need to submit or complete. If you have any questions regarding which form to use, please contact our office for assistance.
Shared Parenting Transportation Requests
Families requesting transportation under a shared parenting arrangement must email berea_trans@bereaschools.org each school year. Please include the following information:
- Student's full name
- Student ID number
- Both home addresses
Childcare Provider Transportation (Grades K–8)
Elementary families may request transportation to a childcare provider at an address different from the student's residence, provided the childcare location is within the city limits of the student's assigned school.
Middle School families may request a childcare provider at an address different from the student's residence, provided the childcare location is within the school district's limits.
Students may be assigned to up to two different bus routes. It is the responsibility of both the parent/guardian and student to know which route the student is scheduled to ride each day.
Space Available Transportation Requests (Grades K–12)
If space is available on a nearby route, please complete the attached form. Space Available requests are open to students in grades K–12. Approved students will be assigned to the nearest available bus stop to their residence.
Please note that Space Available requests will not begin to be processed until mid-October, after mandatory student ridership counts have been completed.
Polaris Students (Grades 11–12)
If your student will be attending Polaris and requires shuttle transportation to Polaris or transportation home from Polaris in the afternoon, please complete the form below.
Transportation OPT - Out Form
Please complete this form for each student in your household who will not be utilizing Berea City School District transportation services. If your transportation needs change during the school year, eligible students may be reinstated on their assigned bus route at any time.
